@chapal Fathers today are more involved with their children than ever before, and they want to know from the pediatrician how their child is developing. Fathers want to be included and made welcome in the doctor's office. Especially with more women entering the workforce, more fathers are stepping in to provide care and they want to know from the doctor what is best for their children

Dairy Fat May Protect Against Diabetes, Says Biomarker Study
Adults with higher blood and fat tissue levels of three fatty acids — which correlate with intake of high-fat dairy foods — were less likely to develop type 2 diabetes, in an epidemiologic study. The researchers analyzed data from 16 pooled international cohorts with more than 60,000 people from the Fatty Acids and Outcomes Research Consortium (FORCE) project. The participants had baseline measurements of three fatty acids — pentadecanoic acid (15-carbon saturated fatty acid, 15:0), heptadecanoic acid (17:0), and trans-palmitoleic acid (t16:1n7) — that reflect consumption of fat from dairy products such as milk and cheese.

RE: Do NSAIDs Negate Bone Effect of Bisphosphonates?
@chapal At least one incident of osteoporotic fracture occurred in 440 women during the follow-up period, which was the same for both NSAID users and nonusers (mean, 2.77 years). Hip fractures accounted for 110 of these fractures. Among the placebo group, the rates of osteoporotic and hip fractures did not differ between the groups with (9.5%) or without (9.7%) NSAID use. Among the women treated with clodronate, however, NSAID use was linked to a 49% higher risk of osteoporotic fracture (P=0.019) after adjustment for confounders.

RE: Platelet-Rich Plasma: Evolving Role in Plastic Surgery
@rohit Despite a lack of standardized protocols for platelet-rich plasma preparation and a scarcity of large-scale studies with long-term follow-up, there is convincing evidence with objective measurement modalities that display positive outcomes after treatment for skin rejuvenation, hair regrowth, wound healing, and fat graft take.

RE: SARS-CoV-2 mRNA Vaccine Might Trigger Corneal-Transplant Rejection
@sudeshna Clinicians and patients should be aware of the potential of corneal graft rejection associated with vaccine administration and may wish to consider vaccination in advance of planned non-urgent keratoplasties. Patients should be counseled on the symptoms and signs that require urgent review to allow early treatment of any confirmed rejection episode.

Prognosis of alcoholic hepatitis?
Alcoholic hepatitis is a syndrome of progressive inflammatory liver injury associated with long-term heavy intake of ethanol. Patients who are severely affected present with subacute onset of fever, hepatomegaly, leukocytosis, marked impairment of liver function (eg, jaundice, coagulopathy), and manifestations of portal hypertension (eg, ascites, hepatic encephalopathy, variceal hemorrhage). However, milder forms of alcoholic hepatitis often do not cause any symptoms.

RE: Prognosis of bedbug bites?
@chapal The prognosis for bedbug bites is excellent, although these bites can create considerable anxiety and localized and occasionally systemic reactions. Sometimes, if the bite reactions are intensely pruritic, scratching with excoriations may be complicated by impetigo. In addition, bedbugs may be a vector for hepatitis B. Of note, reports have indicated the risk of insect transmission of human immunodeficiency virus (HIV), if any, is extremely low and likely nonexistent.

RE: Feeling nausea in the morning, what should I do?
@samik Nausea and vomiting are not diseases, but they are symptoms of many conditions such as:

Medication-induced vomiting
Intense pain
Emotional stress (such as fear)
Gallbladder disease
Food poisoning
Infections (such as the "stomach flu")
Overeating

Management of the Burn Patient?
A 58-year-old female patient was activated as a Level 1 trauma alert after being involved in a house explosion with resultant fire. She was awake and alert with no loss of consciousness at the scene but sustained significant thermal injuries per EMS report. She was intubated pre-hospital for “airway protection” out of concern for inhalation injury with facial burns. Initial evaluation revealed an older woman, orally intubated with bilateral breath sounds, mildly tachycardic in the 100s, moderately hypertensive in the 160s/90s, with readily apparent full-thickness burns to the face, neck, anterior torso, bilateral arms, and bilateral legs (Photos 1 and 2). Secondary survey and imaging revealed no further injuries.

RE: Scabies skin infection in girl (11 years old)!!
@riya Scabies is a type of skin disease which are caused by an infestation and sensitization by Sarcoptes scabiei var. hominis and its product. There are many factors which are contributing to this type of skin disease such as poor hygiene, poverty, and sexual contact and wrong diagnosis. The main route of transmission of this disease is by direct skin to skin contact. Take handshakes and sleeping side by side for an example. Scabies also can be transmitted by indirect contact such as sharing of personal belongings. Scabies is usually spread by female Sarcoptes scabiei mites who reproduces and form the larvae. The life circle of this mite from an egg evolving into adult mite takes approximately 8-12 days. The treatment for scabies comprises of two group of medication, which are topical and systemic medication. Scabimite cream contains permethrin 5% and its recommended as the first choice of topical treatment for scabies because it has less toxic compared to Gameksan but the effect of the medication is same.
