Delusions of parasitosis (DoP) manifest in the patient's firm belief that he or she has pruritus due to an infestation with insects. Patients may present with clothing lint, pieces of skin, or other debris contained in plastic wrap, on adhesive tape, or in matchboxes. They typically state that these contain the parasites; however, these collections have no insects or parasites. This presentation is called the matchbox sign, or what the authors term the "Saran-wrap sign."

@kukum All patients in one region of northern Italy diagnosed with ALS from January 1, 2007, to December 31, 2011 (n = 712) were eligible for the study. Of these, 650 (91.3%) were included in the analysis. Researchers assessed disease severity with the ALS Functional Rating Scale revised (ALSFRS-R). The decline rate was the mean monthly number of points on the score lost from symptom onset to the time of diagnosis. Investigators also collected information on smoking status at the time of diagnosis. Of the total, 18.6% were current smokers, 28.0% were former smokers, and 53.4% were never-smokers. The study found that current smokers were younger at disease onset (64.9 years) than former smokers (67.6 years) and those who had never smoked (66.3 years).

More than a third of men seeking infertility-related care are seeing providers other than urologists, but telehealth offers an affordable, viable option for many men, according to two studies presented at the virtual American Society for Reproductive Medicine (ASRM) Scientific Congress. Yet many men do not have access to reproductive urologists, which is where telemedicine can play a role, according to Alex Zhu, DO, a second-year urology resident at Michigan Medicine in Ann Arbor. In his abstract presentation on October 19, he cited a 2016 study that found that 13 states had no male reproductive urologists and that Kentucky had only one assisted reproduction center. Due to the lack of andrologists, patients often have to drive far distances to see a reproductive specialist or will rely on general urologists who may not have the interest or expertise in male reproduction, It is reasonable to assume that this geographic barrier significantly impacts the access and outcomes of assisted reproductive care. Overall, telemedicine is an invaluable resource for male infertility patients.

Freezing the posterior nasal nerve can reduce the symptoms of chronic rhinitis, researchers say. In what they believe to be the most rigorous study yet of cryotherapy for chronic rhinitis, the researchers showed that the ClariFix cryotherapy device (Stryker) was more efficacious than a sham procedure in improving the symptoms patients reported on questionnaires.

Penicillamine and doxycycline are ineffective. Cutis Laxa Internationale is an international support group for patients with cutis laxa (elastolysis).

@kaberi Speech and language therapy is widely available for people who stammer, although the level of service and waiting times vary across the country. Some treatments, such as feedback devices, are also there. Indirect therapy is where parents make changes to the way they communicate and the home environment, rather than focusing directly on their child's talking. If your child is under 5, this is probably the approach your therapist will suggest you try first. However, if a young child has been stammering for several months and it seems to be getting worse, it may be best to start direct therapy straight away. Indirect approaches are often based on the concept that children start to stammer when they cannot keep up with the demands made on their language skills.

@moucheta The management of acute severe ulcerative colitis depends on early recognition of the unwell patient with colitis, the prompt initiation of treatment and objective assessment of the likelihood of medical failure. This deters ‘hopeful expectation’ in an attempt to avoid surgery. Intravenous corticosteroids remain first-line therapy but are completely effective in only 40%, partially effective in 30% and around 30% come to colectomy. The decision to use ciclosporin or infliximab for those with a poor response to steroids should be made at an early stage, often 3 or 4 days after starting intensive therapy. Decision-making is becoming more difficult with agents such as visilizumab, tacrolimus and the technique of leucocytapheresis as further options. Nevertheless, intravenous corticosteroids and timely colectomy have reduced mortality from nearly 30% to <1% in specialist centres. Ciclosporin has delayed the need for urgent colectomy in many patients, but long-term follow-up suggests the majority come to colectomy within 7 years. Long-term outcome with newer agents, including infliximab, is not yet known.
