    • No Need for Intensive Follow-up of Endometrial Cancer

      Women with endometrial cancer do not need to undergo intensive follow-up, say experts.
      Results from the TOTEM trial show that intensive follow-up does not improve overall survival (OS) compared to a much less intensive surveillance approach, even for high-risk patients.
      "[T]here is no need to routinely add vaginal cytology, laboratory, or imaging investigations to the MIN [minimalist] regimens used in this trial," conclude Paolo Zola, MD, and colleagues on behalf of the TOTEM collaborative group.
      The study was published September 1, 2022 in the Journal of Clinical Oncology.
      In an accompanying editorial, Amanda Fader, MD, Johns Hopkins School of Medicine, Baltimore, Maryland, and colleagues endorse the authors' recommendations.

    • Atypical Presentation of Monkeypox Creates Confusion in Primary Care

      If you haven’t seen a case of monkeypox yet, this is likely to change. In a press briefing on July 13, the Pan American Health Organization (PAHO) announced that there had been a surge in monkeypox cases and that specialists are predicting an even greater increase. On Saturday, July 23, the World Health Organization (WHO) declared the disease a Public Health Emergency of International Concern. Nearly 14% of all monkeypox cases recorded worldwide are concentrated in the Americas, mainly in the United States and Canada. However, the increase in cases observed in Brazil in recent weeks was also highlighted in the speech made by PAHO Director Carissa Etienne, MD, MPH.

    • Too Little, Too Much Sleep Tied to Impaired Cognition

      Short and long sleep duration in older individuals are both associated with impaired cognition, new research suggests. However, imaging data reveal only short sleep is linked to greater amyloid beta (Aβ) burden. In a large, cross-sectional study of patients older than 64 years, Mini-Mental State Examination (MMSE) scores were .08 points lower among those with short sleep duration vs individuals with normal sleep duration, while Digit Symbol Substitution Test (DSST) scores were 1.17 points lower in the long sleep group vs the normal sleep group.

      @kunal Oral antibiotics are indicated in mainly moderate-to-severe inflammatory acne. Tetracyclines and derivatives still remain the first choice. Macrolides, co-trimoxazole, and trimethoprim are other alternatives for acne. The following agents should not be used in acne due to lack of efficacy and safety consideration such as cephalosporins, sulphonamide, and gyrase inhibitors. Tetracycline (500 mg–1 g/day), doxycycline (50–200 mg/day), minocycline (50–200 mg/day), lymecycline (150–300 mg/day), erythromycin (500 mg–1 g/day), co-trimoxazole, trimethoprim, and recently azithromycin (500 mg thrice weekly) are being used successfully in acne. Minocycline and doxycycline are more effective than tetracycline and erythromycin. Recently, doxycycline in subantimicrobial dose (20 mg twice daily) and an extended-release minocycline tablet (1 mg/kg/day) were used and found to be effective, but further controlled trials are needed. Gastrointestinal upset and vaginal candidiasis are most common side effects. Doxycycline can be associated with photosensitivity. Minocycline may produce pigment deposition in the skin, mucous membrane, and teeth. Autoimmune hepatitis, systemic lupus erythematosus-like syndrome, and serum sickness-like reactions occur rarely with minocycline.

      @cherry Applying heat to your abdomen and lower back may relieve pain. A 2018 review found that heat therapy (usually a heat patch or pack) was as effective at treating menstrual pain as NSAIDS. It also may cause fewer side effects. However, the authors noted that more research is needed. If you don’t have a hot-water bottle or heating pad, take a warm bath or use a hot towel. Or, you can make your own heating pad: Cut and sew together two pieces of fabric, leaving a hole at the top.
      Fill with uncooked rice and sew up the hole. Microwave for a few minutes to the desired temperature. Don’t overheat! Let cool, if necessary. Or, wrap your homemade pad in a towel to reduce heat transfer. Reuse as necessary. Learn more about the best heating pads here. You can also purchase a heating pad online. Massage therapy for about 20 minutes can help. Massage therapy for menstruation involves pressing specific points while the therapist’s hands move around your abdomen, side, and back.       @minipaliwal Yes!! It is totally normal. I also used to have such itchy bumps while exercising. You can get itchy red hives on your skin for lots of reasons. The ones that break out when you're sweaty just hot are called cholinergic urticaria (CU). Just consult a doctor. It is normal and can happen to anyone.
